Facelift Introduction

As one progresses through the inevitable aging process, certain physical changes become evident. Because the face is the most readily seen part of the body, these aging shifts are most easily seen there—such as wrinkles, sagging skin, excess fat deposits, and other unsightly developments. Many people attempt a variety of techniques to avoid or delay the aging process, with varying degrees of success.

Fortunately, it is possible to rejuvenate one’s facial features and retain or regain a more youthful appearance. Age-related changes can be “rolled back” or diminished through a facelift procedure, which has a wide range of benefits compared to other cosmetic surgeries. What is a Facelift?

For most facelift procedures, flaps of skin on either side of the face are surgically opened and pulled back, tightening the tissue and altering the contours of the underlying muscle and fat deposits to bring about a more youthful appearance. Once the skin is adjusted, excess skin and unwanted tissue is also removed before sutures are applied for recovery.

These procedures are often done in conjunction with a necklift, helping to reduce jowls or sagging skin under the chin.   

Most non-surgical alternatives such as dermal fillers and fat injections deal more with superficial signs of aging, focusing on wrinkle reduction and provide less drastic and less permanent results. We do offer a non-surgical facelift as well. For those seeking considerable youthful rejuvenation that can last up to 10 years, facelift surgery is well worth the investment.

Facelifts are quite safe, being a routine procedure that our surgeons have performed countless times over the years. Of course, there are inherent risks, as with any major cosmetic surgery, but all patients must undergo a rigorous medical and lifestyle screening to ensure they are healthy candidates for the procedure (and will see optimal results from the surgery).

Types of Facelifts

There are numerous types of facelift techniques, each offering a variety of results, while also differing in the recovery time. 

Surgical facelifts include:

  • Mini-Face Lift - This is a smaller, less-invasive version of the traditional facelift, employing an s-shaped incision around the ear, focusing on reducing sagging and aging skin around the lower portion of the face.
  • SMAS Face Lift - Also called a SMAS rhytidectomy, this surgical procedure aims to reduce loss of volume in the cheeks and improve the youthful appearance of the lower 2/3rds of the face.
  • Deep Plane Face Lift - Developed in response to ptosis of midfacial structures, this procedure corrects aging facial features and helps fix deep nasolabial folds. 
  • Precision Face Lift (by Dr. Kim) - This procedure utilizes a laser scalpel for more exact tissue targeting and removal,
  • allowing for a more non-invasive surgical approach to the traditional facelift. 

Non-surgical facelifts include: 

  • Vampire Facelift - This technique employs Juvederm dermal fillers to reduce and remove wrinkles, with platelets extracted from the patient’s own blood. 
  • Profound RF - This facelift option uses micro-needles that release radiofrequency energy slightly beneath the skin to rejuvenate the patient’s facial features.
  • Thread Facelift - A large needle is passed under the skin, with a thread that has small hooks and graspers that tug the tissues into a firmer position while remaining less invasive.
  • Facial Definition - Dermal fillers and botox are injectable facelift options that do not require surgical intervention, reducing wrinkles and replacing lost fat and collagen deposits.

Surgical vs. Non-Surgical Facelift

A surgical facelift is a more invasive procedure requiring greater preparation and recovery time than non-surgical options. Facial tissue is cut open and adjusted via scalpel, with the wounds then stitched or taped shut after the operation is complete.

Non-surgical facelifts are primarily done through site injections of fillers or fat tissues to fill out wrinkles and surface areas of the face, though chemical peels are usually placed in this category. The results are not as significant, but the procedures can be repeated numerous times to maintain or improve one’s youthful appearance.

The average surgical facelift can provide results that last up to 10 years, while non-surgical facelifts must be repeated every 3-6 months in order to see continued improvement. Of course, in either case, aging will eventually take its toll.

What does a Facelift procedure involve?

To prepare for a facelift procedure, patients must meet with the surgeon beforehand in order to review medical history, determine their current health status, review their facial features, and determine their goals and establish realistic expectations of the results. 

Facelifts can be done in both our hospitals and outpatient facilities, and are normally performed with general anesthesia—though sedation and local anesthetics are options for some patients. Normal procedures average 2-4 hours, though this can be extended if combined with other cosmetic surgeries, such as a neck lift. 

It is normal for patients to experience a level of pain and discomfort after a facelift, with minimal swelling and bruising, as well as drainage from the incisions and numbness of the affected facial areas.    

During a traditional facelift procedure, incisions are begun around the temples and end behind the ears in the lower scalp. The skin is then lifted and tugged into the desired position to obtain optimal results, while excess tissue is removed from underneath. Fat deposits can be reshaped and repositioned to allow for more youthful contouring. Underlying muscle is tightened and the removed skin is replaced, while extra skin is cut away, and the wounds are stitched or taped shut.

Patients should follow all doctor instructions to prepare for surgery in order to minimize risk and improve results. This can include:

  • Reducing medications - Doctors discourage the use of any blood thinning medication at least two weeks before the procedure is scheduled to take place to reduce the possibility of excessive bleeding during the surgery.
  • Hygienic preparation - Patients are provided with a germicidal soap that they should wash their face and hair with the morning of the surgery.
  • Dietary adjustment - The night before the facelift, patients are asked to stop eating, starting at midnight, though water and prescribed medications are still allowed. 
  • Post-surgery assistance - For patients who are undergoing a facelift in an outpatient facility, it is necessary to have someone else available to drive them home and provide company during the first night following the surgery. 

Other specific instructions are provided to patients by their doctors and should be adhered to as strictly as possible.

Properly done, facelifts can improve one’s appearance, reducing visible signs of aging or taking years off one’s visage entirely. Results will vary by patient, taking factors such as self-care routines and lifestyle choices (such as diet, smoking, and alcohol consumption) into consideration. Results are not permanent. On average, results will last for up to 10 years before facial skin begins to show signs of drooping or sagging again.

Who is a good candidate for a facelift?

Many people can be good candidates for facelifts. Foremost, if a patient has deep wrinkles and lines around their mouth, eyes, and nose, these may be minimized via a facelift. Those who have lost collagen deposits in their cheeks, forehead, and lower face are also prime candidates for facelifts. Anyone experiencing laxity of the skin around their face and neck may also wish to undergo this procedure.

It is recommended that patients be relatively healthy and in a normal weight range, as obesity can undermine the results or the length of time in which the results remain visible. There is no specific age that would eliminate someone as a potential facelift patient, but the average age range for most patients is between 35 and 70 years old. Much older, and health complications can be an added risk.

If you are seeking alternatives for facelifts, aside from nonsurgical options, there are numerous non-invasive options, including:

  • Light therapy
  • Chemical peels
  • Dermal fillers
  • Botox
  • Ultherapy
  • Laser skin resurfacing

Despite common beliefs, women are not the only demographic interested in or relevant to facelift procedures. Men are quite capable and often do undergo facelift surgery in order to alleviate signs of aging and address significant jowls or sagging skin around their facial areas. The procedure is the same, no matter the gender, and can restore a youthful, healthy appearance to both men and women.

What are the risks and side effects?

Our doctors are highly trained and experience in facelift surgical procedures, and do everything they can to negate or minimize any risks or complications. However, as with any invasive medical operation, facelifts do come with inherent risks, though most of these can be easily managed with proper hygiene, medication, and post-operative care.

Initial risks during the surgery itself include:

  • Excessive bleeding - Doctors recommend avoiding any blood thinning pain medication for several weeks both before and following the surgery to avoid excessive blood loss. 
  • Infection - Proper washing and care of the incision areas during healing can prevent infection onset, especially if seepage is occurring. 
  • Anesthetic Allergy - A full medical review is necessary to detect any potential allergic reactions to the anesthetic used during the facelift surgery.

After the procedure, certain risks can be posed that can alter one’s facial features, such as:

  • Hair and/or skin loss - Some patients experience temporary or permanent hair loss in or around the incision areas, which can be fixed with hair transplant procedures. If the blood supply to facial tissues is interrupted during the surgery, facial skin can also be lost afterward, though this is rare and can be alleviated with proper wound care. 
  • Hematoma - Normally occurring within 24 hours of the surgery’s completion, hematoma is the pooling of blood under the skin, causing swelling and bruising, and ongoing pressure. This can be alleviated with further surgery before it causes greater pain or complications.
  • Scarring - While doctors go to great lengths to conceal the incisions and resulting scars from a facelift, it is possible for the scars to be visibly red and raised. Cortiscosteroid injections and other treatments are available to minimize this.
  • Nerve damage - While rare, nerve injury can cause temporary, partial, or full facial paralysis, resulting in uneven results. This can often be fixed with further surgical intervention.

The facelift recovery process

It is critical that, following the facelift procedure, patients adhere to any recovery plan established by their doctor. It is entirely normal to experience a range of pain or discomfort in the following days and weeks, which can be reduced with proper medication. Other postoperative symptoms can include swelling, numbness, incision drainage, and facial bruising. However, if a patient experiences chest pain, difficulty breathing, or severe pain that persists despite prescription dosages, they should contact their doctor immediately for further consultation.

For the week after surgery, patients should rest with their head elevated, apply antibiotic ointments as directed, and replace facial bandages according to the schedule your doctor prescribed. Avoid wearing excessive makeup (or even none at all), do not pick at the healing skin, and avoid direct sun exposure for at least three weeks.

Scarring will be minimal in most instances, and will be mostly hidden by one’s natural hairline. Any scars can be further reduced by hair transplants and medical injections, but are often not necessary. 

Because of the bruising and swelling caused by the surgery, patients will not likely see the full results of the facelift for at least a few weeks, until their facial tissues return to normal and heal.

What does a Facelift cost?

Facelift procedure costs will vary widely depending on the patient’s goals in transforming their appearance, the extensive nature of the surgery, the doctor’s breadth of experience, the facility in which the operation takes place, and a wide range of other variables. It will also depend on if the facelift is being performed on its own or is being combined with one or more other cosmetic procedures, which can increase the overall cost. Most insurance companies do not cover facelifts, as it generally falls under a cosmetic procedure rather than anything necessary for health reasons (unless it is being performed to as a corrective surgery following an accident or other medical emergency). For further details, you should contact your health insurance provider to see what they actually will or will not cover when it comes to facelifts.

Before your surgery, your doctor will cover all expense items and provide a detailed quote, as well as offer any information for payment plans or other financing options if needed.
